An exciting opportunity has arisen to join the Outreach Team. This is a dynamic and rewarding role, supporting adults with physical and/or learning disabilities, complex mental health issues including Autistic Spectrum Disorders as well as adults who have offending histories both in their own homes and to help them with inclusive integration and participation in their communities.

This is far from a typical role. You will be part of an Outreach model team providing a wide range of supports to adults who may have physical and/or learning disabilities, challenging behaviour and complex mental health issues including Autistic Spectrum Disorders. You will also support adults who have offending histories.
