Social Work Associate

Responsibilities: To accompany residents to banks (for deposit/withdrawal of money, replacement of bank book, etc.), Housing & Development Board (for returning of keys, retrieving of personal belongings from rental units, etc.) and as determined by the case. To maintain records to facilitate submission of the periodic returns and ad hoc reports for accountability purposes and as per request. To liaise with the various government agencies, Agency for Integrated Care, restructured hospitals, and other social service / healthcare organisations with regards to cases, transport co-ordinations, and medical appointments. To assist in the day-to-day operations and maintenance of the Medical Social Work Department. To provide support to Medical Social Workers (e.g. case management, social-emotional support / practical assistance for residents/next-of-kin).
