Social Worker / Advanced Social Worker - Yate

As an Adult Social Worker or Advanced Social Worker, you will play a key role in improving the lives of vulnerable individuals in our community, supporting them to live as independently and meaningfully as possible. You will work closely with people who need social care, along with their families and carers, to promote independence, ensure outcomes are clearly defined, and regularly monitored and reviewed.

The Hospital Social Care team is made up of social workers, occupational therapists, and social care practitioners. We apply the Three Conversations model to support individuals following hospital discharge, carrying out assessments and conversations in a range of settings, including care homes and rehabilitation units across the wider area.
