A.
Administrative and Scheme-Related Support - Referral Intake and Screening, collaborate with Registered Nurse to conduct intake screening for resident. - Complete social assessment for general understanding of family situation - Pick up cases which require further psychosocial-spiritual support - Admission - Touch base with new resident to check in on transition coping - Respite Care / Receive urgent / planned respite cases - Work with team to ensure admission and work with relevant agencies and families on long term plan e.g. permanent placement in NH or discharge arrangements - Assist residents in applying for government financial assistance programmes, including: Interim Disability Assistance Programme for the Elderly (IDAPE) Eldershield Claims Medical Fee Exemption Card (MFEC) Medifund ElderFund CareShield Life / MediShield Life Home Caregiving Grant PGDAS Silver Support Scheme Other financial schemes as applicable B.
