Social Worker - Behavioral Health Interdisciplinary Program (BHIP)

Summary The VA Northeast Ohio Healthcare System is recruiting for a Social Worker with the Behavioral Health Interdisciplinary Program (BHIP). The incumbent functions as a member of the BHIP team in providing for the biopsychosocial assessment, DSM diagnosis, and treatment of veterans who require behavioral health interventions. Responsibilities The Social Worker duties include, but are not limited to: Performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Conducts psychosocial assessments and develops treatment plans in collaboration with the veteran/family and with the BHIP treatment team. Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs.
