Social Worker (CTPLD)

We have an exciting opportunity for an experienced Social Worker to join our Community Team for People with Learning Disability (CTPLD). This is a full‑time permanent role with hybrid working, salary £40,192‑£44,012 per annum plus an annual retention bonus of £3,000 subject to eligibility.

The Role

You will be working as a Social Worker principally to support young people in transition from Children’s to Adult Learning Disability services.

Your Role Will Involve

• Located within a multi‑disciplinary community learning disability team, you will support young people with learning disabilities and their families and carers to make a positive transfer to adult services, liaising with Children’s Services, Health Services, Education, Care Agencies and other organisations.
