Job Description Working in a hybrid style, based at home and Claverhouse office, you will work full time, 37 hours per week on a fixed term basis for 2 years. Responsibilities You will assist in maintaining vulnerable people in the community by providing a range of care management tasks. These include the assessment of individual need, the negotiation of flexible packages of care, and the monitoring and reviewing of individual care packages. The Individual You will have knowledge of Adult Support and Protection legislation. Working knowledge of assessing needs with experience and knowledge of social work values, in working with long term conditions, experience of working with other professionals and involvement in associated areas of interest/voluntary organisations and third sector agencies.
