Homes Not Hospital Job Advert- KSG

The Homes not Hospitals Team works to discharge plan from hospital for people with a Learning Disability and/or Autism who have been identified as part of the Homes Not Hospitals cohort.

We are looking for a KSG Social Worker to fill a permanent part-time post (22.2 hours per week).

You will play a key role in delivering the local authoritys statutory duties, including assessing eligible social care needs, undertaking Mental Capacity assessments, completing community DoLS applications, assessing Section 117 needs and acting as social supervisor where necessary (training will be given). Utilising strengths and rights-based approaches, you will support people to reintegrate into their communities, reduce inequalities, improve quality of life, instil hope and realise real change.
