Social Worker – Inpatient Medical/Surgical Unit

Job Summary:
In this position, the Social Worker is designated to inpatient medical/surgical units at the downtown campus. The Social Worker provides comprehensive psycho-social assessment to identify and address social drivers of health while working collaboratively with the multidisciplinary team to support the needs of patients and families. The candidate shall gain knowledge of social determinants, population health, and drivers of utilization. Duties will include rounding with the multidisciplinary team on assigned units, consultation including assessment, brief counseling, and intervention. Candidate shall learn to initiate and implement advance directives and will assist with discharge planning recommendations and referrals to meet the psycho-social needs of patients and families.
