Rate: £38 per hour Ltd. or £35 per hour with £150 weekly expenses (for those living outside of West Sussex)

Liquid Personnel is seeking a dedicated qualified Social Worker to join our client’s Mental Health Hospital Discharge Hub based in Horsham.

What will your responsibilities be?
In this role, you will complete all statutory work such as Care Act assessments, s117 aftercare planning meetings, safeguarding, Mental Capacity Assessments, and Best Interest Meetings. You will work proactively under pressure to ensure all aspects of the individual’s needs are considered and incorporated into the discharge process.
