Social Worker – Hospital Discharge Team

Location: Plymouth
Hours: Monday - Friday
Working Hours: 8:45am – 5:15pm
Pay Rate: £25.38 per hour (PAYE), paid weekly every Friday
Start Date: ASAP
Contract: Temporary
On-site Only: No remote working

We are recruiting a Social Worker to join the Hospital Discharge Team in Plymouth. The role supports patients admitted to hospital, ensuring safe and timely discharge planning through appropriate pathways.

This is a fast-paced, on-site role requiring full-time presence within the hospital and active ward-based working.

Key Responsibilities

  • Complete timely, person-centred assessments and care planning
  • Support safe discharge planning, including D2A pathways
  • Work within a multidisciplinary team (MDT) and partner agencies
