Job Summary

Liquid Personnel is seeking an Adult Social Worker to deliver high-quality, person-centred social work within a multi-disciplinary rehabilitation team based in Bromley. The role involves supporting adults with neurological conditions and those transitioning from hospital settings under the Discharge to Assess (D2A) model. It focuses on timely assessments, care planning, and promoting recovery, independence, and safe discharge.

Responsibilities

• Undertake Care Act 2014 assessments and develop strengths-based support plans for individuals with neuro-rehabilitation needs or post-hospital discharge.
• Work collaboratively with occupational therapists, physiotherapists, nurses, and hospital discharge teams to ensure safe and effective transitions from acute settings.
• Complete Mental Capacity Assessments, Best Interest decisions, and contribute to DoLS processes where required.
