Opportunity Description
We are seeking a Social Worker to provide professional casework and psychosocial support to individuals and families with complex needs. The role will support Conferences in accompanying Friends-in-Need (FINs), while contributing to case management, community partnerships, and service delivery initiatives.
Key Responsibilities- Provide end-to-end case management, including assessment, care planning, intervention, review, and case closure for individuals and families with complex needs.
- Conduct psychosocial and risk assessments, identify key issues and protective factors, and develop appropriate intervention plans guided by professional standards and ethical practice.
- Support Conferences in reviewing client needs, coordinating referrals, and liaising with community partners and social service agencies to facilitate holistic support for FINs.
