STATUS: Per Diem LOCATION: Golisano Rehabilitation - 1555 Long Pond Rd. Rochester, NY DEPARTMENT: Rehabilitation/Speech Pathology SCHEDULE: Day shift, 8-hr. shifts, 8:30 am - 4:30 pm (per diem as assigned)

RESPONSIBILITIES:

• Evaluation & Planning. Perform a complete initial evaluation of the referred patient; develop and implement treatment plans, including appropriate short- and long-term goals; involve the patient, family, and staff in the implementation of the treatment program; re-evaluate patient functional status progressively; advance patient toward maximal performance and functional independence
