Opportunity Description
The Community Inpatient Unit provides rehabilitation, re‑ablement and recovery interventions for adults in North East Lincolnshire. The service promotes faster recovery from illness, prevents unnecessary acute hospital admissions, premature admissions to long‑term care, supports timely discharge from hospital and maximises independent living wherever possible. Multi‑professional staff collaborate with individuals to ensure they achieve their full potential in terms of their level of independence, health, and wellbeing.
CIU provides short‑term re‑ablement for Service Users who may need to regain their confidence and skills on discharge from hospital or to get over a short‑term health exacerbation. This includes assessing complex and specialist needs and developing, implementing and evaluating programmes of care, in order to support individuals to remain in community settings, in their own home, residential home or nursing home.
