Stroke Wellbeing Coordinator: Care & Navigation

The NATIONAL NEUROSCIENCE INSTITUTE OF SINGAPORE PTE LTD is seeking a healthcare professional to support the NNI Stroke team in implementing patient and caregiver screening. You will establish rapport with patients and coordinate care services.

This role requires a recognised diploma, proficiency in Microsoft Office Suite, and an advantage with Microsoft Access knowledge. You will assist with screening needs and follow-up on patient conditions through phone calls or home visits.
