Substance Use Case Manager - Middlebury

Lund’s Regional Partnership Program (RPP) seeks a dedicated and driven Case Manager to support families, create meaningful change, and address substance use challenges in our communities.

RPP Case Managers work out of local DCF Family Services offices to improve the well-being and permanency outcomes of children affected by parental substance misuse.

This Middlebury-based position will primarily support the Middlebury DCF district office while also providing coverage and support to additional districts as needed.

Flexibility, collaboration, and the ability to build strong working relationships across multiple communities and teams are essential in this role.

In close collaboration with DCF staff, RPP Case Managers play a vital role at the front end of child protection, screening for problematic substance use, linking parents to appropriate treatment services, and addressing barriers to successful engagement.
