Job Summary:


In addition to the responsibilities listed below, this position is also responsible for leading team to aid development, maintenance, and implementation of complex clinical practice standing orders, drug therapy management protocols and guidelines (e.g., collaborative protocols with providers); proactively supervising team to provide complex direct patient care, drug utilization, and educational programs related to ambulatory care to positively impact patient adherence and outcomes and provider prescribing practices; supervising team and may also provide direct patient care for medication initiation, medication selection, dosage titration, drug monitoring, patient engagement, medication adherence counseling, side effect management, medication discontinuation in alignment with strategic priorities and mission work; regularly maintaining and developing day-to-day relationships with providers to align drug conversion, drug tapering, and discontinuation initiativ...
