This is a key leadership role for an RN, LPN, or Case Manager with strong clinical expertise and hands-on Swing Bed experience who thrives in a collaborative, patient-centered environment. The Swing Bed Coordinator is responsible for overseeing and taking operational ownership of all aspects of the hospitals Swing Bed Services, including daily census management, CMS compliance, documentation accuracy, referral management, interdisciplinary coordination, and program financial performance to ensure high-quality patient outcomes and operational success.
