About the Role

We are seeking an experienced and passionate Team Coordinator to lead and support our Health of Older People (HOP) team within Support Net, delivering essential Needs Assessment and Service Coordination (NASC) services to our community. This is a unique opportunity to combine clinical expertise, operational leadership, and people development in a role that directly impacts the wellbeing and independence of older people and individuals with disabilities.

Key Responsibilities

• Provide day‑to‑day operational leadership and coordination of NASC services.
• Support and coach Needs Assessors and Service Coordinators.
