Transfer Agent - Per Diem - Varies - Mobile Health

Responsible to assist Critical Care Transfer Nurses in providing safe, efficient, and legal coordination of patient transfers between acute care facilities in a fast-paced, 24/7 call center environment.

+ Answer incoming calls in a timely and professional manner

+ Convey accurate messages to RNs in a timely manner

+ Obtain information to complete an initial intake

+ Make calls to sending/receiving facilities to request/collect face sheets and demographics

+ Send/Received faxes & email

+ Use a variety of office equipment

+ Call and arrange the correct mode of transports for patients

+ Highly proficient in data entry and typing skills

+ Prepare various reports, case audits, and other duties as assigned

+ Assist with training of new hires

Required:

+ Highly proficient in Microsoft Office applications
