Transfer Center Analyst

Job Description
A large hospital system is hiring an overnight (7:00 PM–7:00 AM) Transfer Center Coordinator, with a schedule of two 8-hour shifts and two 12-hour shifts, including every third weekend. This person will be responsible for coordinating all aspects of Transfer Center and telemedicine cases including facilitating physician to physician communication and logistics related to patient transfers. This includes scheduling and assigning transports, documenting transfer activity, facilitating communication for transfer and completing ADT functions in Epic.

Job Responsibilities:
• Receives calls regarding patient transfers, consultations, and telemedicine requests, and obtains pertinent clinical and diagnostic information from the referring physicians, RNs, and other clinical staff. Coordinates physician to physician communication.
