Transition Social Worker - Youth with LD (Hybrid)

Royal Borough of Windsor and Maidenhead is seeking an experienced Social Worker to join our Community Team for People with Learning Disability. This full-time permanent role offers a salary of £40,192 to £44,012 per annum and an annual retention bonus of £3,000.

The successful candidate will support young people transitioning from Children’s to Adult Learning Disability services in a hybrid working environment. Key responsibilities include conducting assessments, care planning, and safeguarding investigations.
