Overall Job Responsibility
In this role, you will develop holistic care plans through comprehensive bio‑psycho‑social assessments of families through a person and family‑centric approach. You will provide case coordination to safeguard the long‑term welfare and financial security of persons with disabilities, including facilitating future care planning (FCP), setting up of SNTC Trust where relevant, and conducting regular reviews of care plans for SNTC Trust beneficiaries.
Responsibilities- Engage families in FCP conversations and help them plan for persons with disabilities' long‑term care needs.
- Assess eligibility of clients for sponsorship schemes and facilitate applications.
- Assess eligibility and suitability of the Special Needs Trust for the client and facilitate Trust setup accordingly.
