Wellness Coach - Community Health Education

**Description**



Under the supervision of the Executive Director, the Wellness Coach will work closely with Regional Health Education as an extension of the care teams supporting patient's goals. This position works independently providing coaching, educational support, resource navigation and troubleshooting by phone, in person, group and individual settings. In addition, a regional resource for both patients and employees navigating all wellness/disease management programs within or outside St. Joseph Hoag Health. Partner with SJHH Employee Wellness program to extend the education/disease management services at the employee worksite.
