YR23- Social Worker - Clementi / Telok Blangah / Degree Holder / Experience Needed /Daycare Centre

Salary: $3500 - $4500
Working Days: Mon – Fri (40hrs/week, center operations hours from 8am to 6pm)
Working Location: Clementi / Telok Blangah

• Review applications from AIC. Interview clients and caregivers during enrolment into our programme/home. Conduct Biopsychosocial assessment to assess the strengths and resilience of the client, families, and social support systems.
• Build rapport with residents/clients and their families. Maintain constant communication with clients while addressing their concerns and goals, helping them keep a positive attitude and educating family members on the physical and psychosocial needs of the clients and ways they can access internal and external resources.
• Provide casework management for our clients and coordinate with external agencies such as hospitals and polyclinics for clients who require financial assistance for their medical appointments.
